About Breast Size

Though breast size is largely determined by genetics, a woman can see her breasts fluctuate in size through different stages of her life. For many, breast size plays a vital role in their sense of attractiveness and femininity.[2] Unfortunately, breast size can be a major source of insecurity (70% of women say they are unhappy with the size and shape of their breasts).[3] While other areas of the body respond to diet and exercise, breast size is impossible to control. Breast augmentation has been shown to improve patients’ body image and significantly impact their overall well-being.[4]

About Breast Augmentation

During your consultation, our team at Plastic Surgeons of Lexington will ask different questions to better understand your aesthetic concerns regarding your breasts. We will take measurements and perform an exam to plan for the most ideal implant that meets your aesthetic goals.  Modifications for implants include:

Composition

Breast implants come in either saline or silicone. Silicone implants are more popular, making up 84% of the implants used in 2020[1] and 98% of the implants used in our practice. Silicone implants better mimic breast tissue, creating a natural look and feel to the breast. Silicone implants are safe, do not cause concern or autoimmune disease. Also, importantly, they do not deflate like saline implants. Saline implants have a different consistency and often lead to more rippling, as well as a higher rupture rate. However, silicone implants do need to be monitored by ultrasound after the first three years and every two years after, according to the FDA. 

Size

The size of the implant usually is one of the most important factors when determining breast implants. Instead of cup sizes, breast implants are measured in cubic centimeters (cc’s). Typically, breast implants range between 120cc and 800cc. A smaller implant can be utilized for women wanting only a subtle, moderate change, while women who want a more enhanced change can opt for an even larger implant. There are also different profiles to implants available. This determines the projection of the breast from the chest. Higher profile implants extend further for a more dramatic effect, while lower profile implants have a more subtle appearance. Our goal is to meet your desires optimally but also utilize an implant that fits your unique breast dimensions and characteristics. 

Shape

Breast implants come in either a round or teardrop shape. Teardrop implants are more anatomically shaped and have a slight slope to them, resulting in a natural-looking, subtle appearance. Round implants can create a fuller breast with less implant volume. Choosing the right shape is vital in achieving your desired appearance – a task only trained plastic surgeons can properly assist you with. 

Incisions

There are three incisions that are utilized in breast augmentation: inframammary crease, periareolar, and axillary. The inframammary (IMF) incision is the most widely utilized. It gives the most direct access to the pocket, thereby limiting complications such as implant malposition and bleeding.

The periareolar incision has a much higher risk for capsular contracture and therefore is not utilized.

Placement

Implants can be placed beneath the gland (subglandular) beneath the fascia of the pectoralis muscle (subfascial) or beneath the muscle (submuscular). Submuscular positioning of the implant is the most common method, as it has been shown to greatly diminish the risk for capsular contracture (breast hardening). Breast subfascial augmentation too has been shown to diminish capsular contracture by a similar degree, but the body of evidence is not as robust as the evidence for submuscular positioning. Subglandular augmentation is typically performed in concert with a textured surface implant, which helps reduce the risk of capsular contracture, due to association with anaplastic large cell lymphoma of the breast textured surface implants have been largely abandoned and in some instances are recalled.  Therefore, subglandular augmentation is not a satisfactory implant location at this time. 

Candidates

Patients interested in breast augmentation are invited to schedule a $100 personal consultation that is only applicable for this surgery. Women who are good candidates for breast augmentation are:

  • Those desiring larger and more full breasts
  • Non-smokers or able to stop smoking a couple of weeks before their procedure
  • Otherwise healthy, without major heart, lung or other medical problems 
  • Cleared by mammogram if 40 years of age or above

We will ask you questions about your ideal appearance and cup size. We will perform an exam and match you to your ideal implant while taking into consideration your desired size and shape. A sizing system will help guide you in determining the volume implant you prefer.

Preparation

To prepare for your procedure, it is important to:

  • Avoid blood-thinning medications like aspirin, ibuprofen, Advil, and NSAIDs. These will cause excessive bleeding during your procedure.
  • Stop smoking or using other nicotine products as nicotine constricts blood vessels and hampers the body’s natural healing ability.
  • Designate a caregiver to drive you home. Breast augmentation is performed under general anesthesia, so you will not be fit to drive yourself home post-operation. 
  • Pre-fill all prescription medications that we give you. After your procedure, it will be important to focus only on rest and recovery. 

Procedure

After we have administered the proper anesthesia, the predetermined incision will be made. 

Then, the implant will be placed into the predetermined breast pocket. After the implants are symmetrical and even, the incision will be closed, and your breasts will be wrapped in a compression garment. 

Recovery 

For the next couple of weeks, you will need to wear a soft but supportive bra and continue only taking the prescribed medications for pain. Until your doctor says otherwise, sleep on your back and don’t disrupt the incisions. At your follow-up appointments, we will determine where you are in the healing process and gradually increase your activity level over the ensuing weeks. 

Other Breast Procedures

While many women desire a different shape or projection to their breasts, other women wish to see their natural breasts stay the same size, only perkier. To do this, we perform a breast lift. A breast lift is a surgical solution to ptosis (sagging) in the breast tissue. By removing the excess, saggy tissue from the breasts, we can relocate the nipple and lift the breast higher on the chest. Sometimes, a breast lift is used in conjunction with a breast augmentation for ideal results.

Alternatively, for women who wish to have smaller breasts, we recommend a breast reduction. This procedure surgically removes breast tissue and reduces the size of the mammary glands. Patients who are interested in breast reduction typically experience shoulder or back pain due to uncomfortably big breasts.
